US Immigration Medical Exam Requirements and fees


For a USCIS Certified Civil Surgeon to complete the physical exam form (Form I-693) and clear you for the purpose of obtaining a U.S. permanent residency (Green Card) you will need the following: 

1. A physical examination or a medical check-up

2. Tuberculosis Test (Skin or blood test). If the Tuberculosis test is Positive then you will need a chest X-Ray.

3. VDRL Blood test for Syphilis

4. Required Vaccinations are TDAP, MMR and Varicella. Influenza vaccine needed only between Oct 1st to Mar 31st. Pneumonia and Shingles vaccine only for applicants 65 years or older. Bring all your vaccine records and blood test results for vaccination related antibodies when you come in. f you have received all the vaccines but do not have proof of the vaccination, you can opt for the blood test to check if you have protective antibodies from a prior vaccination in your blood.  

5. Bring your passports or drivers license or a government issued photo ID

6. Completion of all green card physical forms by the USCIS Certified Civil Surgeon after requirements 1-5 are met.


The medical examination fee is $180. Blood tests,vaccinations and X-Rays(if needed) are not included in this fee. If you have health insurance blood tests may be covered under your plan. We do provide vaccines but the rates vary from time to time. You have the option to get the vaccinations from a Pharmacy.
